Purpose: To recognize an individual who exhibits outstanding abilities to enlighten and mentor others in the profession

Award: 

• Certificate of Recognition
• Crystal Object (Not to exceed $50)
• Recognition during Awards Ceremony at ASCLS-Michigan Annual Conference

• ASCLS-Michigan Member

Nomination Requirements:

• ASCLS-Michigan Donna Duberg Mentorship Award Nomination Form (completed online)
• Nomination Letter (emailed to exec@ascls-michigan.org)

Nomination Deadline: March 1

Recipient Selection: Vote of the past 5 recipients of the award
